Medication Guidelines & Medical Forms

If at all possible, Parents/Guardians are advised to give medications at home. Students will be given medication at school when the medication is prescribed or ordered by a physician, after a parent/guardian completes a Medication Permission Form, and if the medication must be given during school hours to maintain sufficient health to participate in the school program. 

*Narcotics will not be given at school 

Failure to adhere to the prescription and non-prescription (over the counter) medication regulations may be considered a violation of the substance abuse policy.  The complete Medication Policy can be found at the link below, in the Augusta County Policy Manual.

Parents should also sign an exchange of information form. The nurses use this to obtain documentation from the physician for medication changes, etc. If you have any questions, please do not hesitate to call the Regional School Health Coordinator, Rebecca Hipes, or ask your clinic aide. Laura Sutton's contact information can be found by clicking her name.

Physician Input Form: Completed by doctor in order to address student medical issues at school. 

Medication Permission Form: Completed by physician for medications given at school. 

Release of Information Form: Used to fax the physician for medication changes (Valley Pediatric Patients will need to complete the Valley Pediatric Exchange form at their office). 

Asthma Action Plan: Used as a guide in the treatment of asthma. 

Food Allergy Action Plan: Plan of action and medication orders, completed by the physician, for students with severe allergies. 

Seizure Action Plan: Plan of action and medication orders, completed by the physician, for students with seizures.
